Community Support Network

The Community Support Network is part of HealthyNYC, a citywide effort to help New Yorkers live longer, healthier lives. We connect individuals and families to critical resources that address:

  • Chronic Disease: Support for heart disease, diabetes, and cancer screenings to reduce preventable deaths.
  • Mental and Emotional Health: Stress, anxiety, and crisis support to lower suicide rates and promote resilience.
  • Substance Use and Harm Reduction: Treatment and overdose prevention to reduce drug-related deaths by 25%.
  • Personal and Community Safety: Resources to promote safer homes, neighborhoods, and overall well-being, aligning with a 30% homicide reduction goal.
  • Maternal and Infant Health: Resources for pregnant individuals, improving birth outcomes and reducing disparities.
  • Food, Housing, and Economic Stability: Connections to food, housing and career pathways for long-term health and security.

Burial Assistance

The Human Resources Administration’s Office of Burial Services helps individuals in need of financial assistance to meet funeral expenses for a deceased low-income NYC resident.

OBS can pay up to a maximum of $1,700 towards the cost of a funeral bill.

Total burial cost cannot exceed a cap of $3,400. To determine eligibility:
